28-year-old woman.
She requests the extraction of the right upper-central incisor.
We explain to her that due to the malocclusion detected (open bite, maxillary compression), placing an implant is not recommended, since an implant treatment is irreversible. Therefore, a previous orthodontic treatment is recommended.
After a corticotomy (decortication for accelerated orthodontics), an orthodontic treatment is conducted in order to correct the malocclusion and the maxillary compression.
Afterwards, the tooth is extracted and an implant is immediately placed together with saddle-shaped bone and gum grafts.
Later, a zirconium-porcelain crown is placed, which provides excellent maintenance of the soft tissue contours.
1.- The right upper-central incisor must be extracted.
Previous corticotomy and orthodontic treatment are required to correct malocclusion.
2.- Right side view. The malocclusion is noticeable. A corticotomy will be performed so that the upper teeth will occlude outside the lower teeth.
4.- Global overview of the upper maxilla, which reveals compression and teeth misalignment. This will be corrected with corticotomy+orthodontic treatment.
5.- End of the orthodontic treatment. At this point, the extraction of the right upper-central incisor and the immediate placement of an implant are planned.
6.- Global overview after the orthodontic treatment. Note the proper alignment of the teeth.
7.- Non-traumatic extraction of the central incisor.
8.- Immediate placement of an implant after extraction.
9.- Saddle-shaped graft of gingival connective tissue to seal the alveolus entrance after placing the implant.
10.- Occlusal view of the saddle-shaped graft of gingival connective tissue stitched.
11.- Zirconium-porcelain crown on implant in the right upper-central incisor.
12.- Side view which reveals an excellent emergence profile of the crown on the implant.
