This class III malocclusion (antero-vertical excess) presented, on top of a dental problem (overcrowding), a skeletal problem at the vertical and antero-posterior level.
The option of a combined treatment of orthodontic appliances and orthognathic surgery was first suggested, but it was rejected.
As an alternative to orthognathic surgery, the patient underwent a treatment involving the extraction of the first upper and lower premolars to solve the overcrowding, the anterior crossbite, the open bite and the sagittal relationship.
